The Tools I Use in a Personalized Approach to Consultation

Hogan Pesaniello, M.D.

 

    My primary clinical interest is in the use of psychotherapy & neurofeedback in addressing the issues brought to me in 1:1 consultation.  Optimum self-regulation requires a well-regulated brain and nervous system.  Mood issues like depression and anxiety, as well as interpersonal problems, behavior issues, and personality issues can all be improved by optimizing self-regulation through a variety of tools. I have found neurofeedback and biofeedback to be potent tools in addressing these as well excellent tools for improving symptoms associated with stress-related conditions like migraine and irritable bowel.  My mode of psychotherapy is growth / process / insight oriented.  I utilize biofeedback, neurofeedback, cognitive work, relaxation training, imagery, dreamwork, and medication management as appropriate.  I have provided group therapy and couples sessions.  I am comfortable utilizing faith-based interventions.  I am not a pastoral counselor, but I respect and value the contributions of faith and community in each individual's journey.

About Insurance:  You may be able to use your insurance to help pay for your treatment.  Dr. Pesaniello does not participate with any insurance companies. She is considered out of network, unless you make arrangements with your insurance company for a special accommodation.  This is sometimes an option due to Maryland Law addressing inadequate provider panels.   Dr. Pesaniello’s staff will assist you in  preparing your insurance claim so you can submit it for reimbursement yourself, or her office can submit it for you.  Refer to the page on our website titled: Dealing with insurance companies for tips on approaching insurance and clarifying your coverage.

Are you blue? Depressed, Anxious, or Angry?

     If you find yourself suffering from constant low moods, or if you keep having trouble in your relationships with your partner or friends or co-workers, you can find help.

Some causes of emotional and relational problems include the following: 

 

  •   Simply needing wise, professional and confidential counsel.

 

  •   Needing skills for handling anger, anxiety, sadness, abuse, etc.

 

  •   Avoiding problems through the use of alcohol, drugs, work-aholism, etc.

 

  •   Having a medical disorder that causes emotional symptoms.

 

  •   Inheriting a tendency to have painful emotions (sadness, irritability) that are excessive, inappropriate, or un-ending.

  • Having habits of thought and behavior that cause distress and dysfunction.

 

  •   Having a history of loss, trauma, abuse, or disappointment that has never been worked through.

Healing can begin in a number of ways:

​

  • A professional assessment to clarify what the problem is, and to obtain recommendations for treatment is a good start.

 

  •  Talk therapy alone, relaxation training, biofeedback, neurofeedback, medication, or a combination of these may be advised.

 

  •   Learning to manage intense feelings, learning to relax, learning healthier thoughts and behavior skills, learning to manage emotions and conflict with confidence can all be a part of therapy. 

 

  •   Once more intense symptoms have lessened, it may be easier to pursue other modes of healing and growth, such as pastoral counseling, spiritual growth, community or recreational participation and exercise.

Do I Need to See a psychiatrist?

 

    Psychiatrists are medical doctors who have completed college, medical school, an internship in general medicine, and a psychiatric residency of three years. 

​

    Psychiatrists are trained to provide evaluation and treatment for a wide variety of emotional / behavioral issues.  All psychiatrists are trained in talk therapy and medical management of psychiatric disorders.  Others also include more intensive psychotherapy training as part of their education.  Some add neuromodulation to their toolbox:  This would include neurofeedback, biofeedback, and stimulation therapies like TMS or ECT.
